Modding, which allows you to add and customise game content, can take your gaming experience to the next level. However, it’s common to encounter problems with mod managers that help you use these mods.

Quick fixes
Here are a few ways to help you fix the mod manager not working issue.
Don’t move the files
Make sure the folders and the .exe file stay together where you first put them. For example, if you initially extracted everything to your desktop, keep all the downloaded files on your desktop. If you put them in a mod folder, make sure they all stay inside that mod folder without moving them elsewhere.
If you have already moved the files, copy them back to the original folder, or you can again extract the files in a new folder and don’t move them this time.
You can make a shortcut of the .exe file and paste it on the desktop to make it easily accessible.

Delete game_version.lsx file
Some players have resolved the issue by deleting the game_version.lsx file. The main reason behind this is GustavDev file was automatically deleted from the folder, you can bring it back by following the steps below:
Step 1: Go to the LocalApp data folder and ensure that your modsettings.lsx file remains unchanged.
Step 2: Extract the zip in its own folder.
Step 3: Run the executable file which will generate a game_version.lsx file in the tool’s folder. This file contains information necessary to adjust your modsettings.lsx for the current game version. Delete the game_version.lsx file.
Repeat the process after every game update.
Step 4: Now, use the BG3 mod manager to apply the mods.

Run as administrator
Sometimes, the mod manager might not work as expected because it does not have enough permissions; running it as an administrator will allow it those permissions. To run it as administrator, follow the steps below:
Step: Right-click on the .exe file shortcut, and then in the dropdown menu, click on the Run as administrator option.
